A case of fibrino-purulent peritonitis due to necrosis of the appendicular stump in an eight-month pregnant woman

Abstract

Of all the complications following the early postoperative period of an appendectomy, the appendicular stump necrosis is one of the most formidable ones. The duality of not well-presented symptoms in the early postoperative period, combined with a rapidly progressing peritonitis, forms a devastating, life-threatening condition. All presented above, combined with a last-trimester pregnancy, create a stage for difficult choices and challenging diagnoses.

Двойнствеността на недобре изразените симптоми в ранния следоперативен период, съчетана с бързо прогресиращ перитонит, създава разрушително, животозастрашаващо състояние. Всичко това, съчетано с бременност в последния триместър, създава условия за трудни избори и предизвикателни диагнози. КЛЮЧОВИ ДУМИ некроза на апендикуларния чукан, апендектомия, бременност, трети триместър ВЪВЕДЕНИЕ По дефиниция некрозата на апендикуларния чукан или апендицит на чукана е патологично състояние, при което остатъкът от апендикса след апендектомията претърпява процес на възпаление или исхемия на тъканите, което води до некроза на този фрагмент, която може да прогресира дори до цекума. По време на бременност, в присъствието на растящия плод, женският организъм претърпява драстични промени с бързи темпове. Промяната е не само физиологична, но и анатомична. Уголемената матка изтласква съседните органи. Цялото дебело черво, а по-точно цекумът и апендиксът, се изместват краниално. Стандартното анатомично положение на органите се променя, което до известна степен замъглява правилните симптоми. Като нормално развитие, цекумът и апендиксът се изтласкват до нивото на десния хипохондриум през последния триместър на бременността. Палпаторните симптоми не се проявяват добре и се маскират от уголемената матка и движещия се плод в нея. Коремният статус и палпаторните симптоми не могат да бъдат приложени, както при стандартен преглед. Типичната апендикуларна болка, която следва апендицита, в повечето случаи се проявява в ново, нетипично място. [ (1,2,3,4) ] Некрозата на апендикуларния чукан е типично състояние, което се развива в ранния следоперативен период. Това е рядко състояние с честота на възникване приблизително 1 на 2000 до 2500 случая. Проявява се с повторно появяване на типичните симптоми на апендикулярна болка и висока температура. Състоянието прогресира бързо до перитонит. Ранното проявление на некрозата на апендикуларния чукан се изявява с внезапна болка в долния десен квадрант на корема, десния фланг на корема и дясното подребрие, висока телесна температура и треска. В повечето случаи, пациентите съобщават за повторно появяване на основните симптоми на апендицит, както преди хирургичната интервенция. Това симптоматично повторно появяване в повечето случаи, независимо от доброто му проявление, е намалено, размито и маскирано от болката, причинена от самата оперативна интервенция. Състоянието бързо прогресира до перитонит и се превръща в животозастрашаващо състояние. ([5,6,7,8,9,10,11,12]). СП. ХИРУРГИЯ • J. Тя се оплаква от болка в горната част на корема с неопределен характер, която е започнала предходния ден. От сутринта в деня на хоспитализацията, болката е локализирана в долната част на корема, главно от дясната страна, и е станала постоянна, придружена от гадене, без повръщане. Субфебрилна температура до 37,7°C. Отхвърлена е патология, свързана с бременността, и е приета с клинични и лабораторни данни за остър апендицит. Пациентката има анамнеза за хранителни алергии към ядки и плодове. От общото състояние: Жена на видима възраст, съответстваща на действителната й възраст. Леко увредено общо състояние. Кожа и видими лигавици - бледи. Език - влажен, покрит с белезникав налеп. Нормостеничен хабитус. Ясно везикуларно дишане, леко отслабено в дясната основа, без хрипове. Ритмична сърдечна дейност. Кръвно налягане - 120/80 mmHg. Корем - над нивото на гръдния кош (31 гестационна седмица), матка на нивото на пъпа, мека. Болезненост при допир на десния латерален кант с пунктум максимум на нивото на дясна илиачна зона, където има и признаци за перитонеално дразнене. Палпаторните признаци на Мендел, Дънфи, Ровзинг и Блумберг са положителни. В целия корем се аускултира забавена перисталтика. Крайници – без оток или трофични промени. От клиничната лаборатория броят на левкоцитите е висок - 13,33x109. Пациентката е оперирана и е извършена лапароскопска апендeктомия. Пневмоперитонеумът е постигнат чрез отворения субксифоиден метод на Хасон. Под визуален контрол е поставен 10-милиметров троакар, позволяващ въвеждането на оптиката. При изследването е установена уголемена матка с аднекси, съответстваща на гестационната възраст. Малко количество мътен серозен ексудат в илеоцекалната област, който се аспирира до сухо. Намери се: гангренозновъзпален апендикс с умерено разширени мезентериални съдове в периапикалната област. Не са открити други видими патологични промени в интраперитонеалните органи, достъпни за изследване. Друг 10-милиметров троакар е поставен в десния хипохондриум, а 5-милиметров троакар е поставен инфраумбиликално и е извършена апендектомия. Мезоапендиксът е прерязан с LigaSure. Апендиксът е мобилизиран, за да се открие основата му. Апендиксът се преряза в основата с с TA 30/3.5 ендоскопски ушивател и Ligasure с последващо отстраняване на препарата през 10-милиметровия трокар в десния хипохондриум. Абдоминалният ултразвук три дни по-късно не показва патология. На петия следоперативен ден пациентката съобщава за поява отново на остра болка в долния десен квадрант на корема. Болката била постоянна и се усилвала с времето. Гаденето се появило отново. От лабораторията – ново повишение на левкоцитите – 10,5x109 и по-високо от нормалното CRP -177,27 mg/L. Пациентката e прехвърлена в специализирана клиника по акушерство и гинекология. Поради прогресиране на симптомите е поискана консултация с коремен хирург. Клиничният преглед показа влошаване на състоянието с положителни симптоми на апендицит: Мендел, Дънфи, Ровинг и Блумберг. Дори симптомите на обтураторния симптом и псоас симптомът бяха положителни. Поради бременността, за диагностика бе използван само ултразвук. СП. ХИРУРГИЯ • J. Чрез извършеното Цезарово сечение на матката се роди едно живо дете от мъжки пол. Процедурата продължи с хирургичния етап на интервенцията. Матката бе покрита с антисептични компреси и се направи екплорация на коремната кухина. Установено бе състояние след лапароскопска апендектомия, с тифлит и перитифлитни промени в цекума. Същите промени са установени на висцералния перитонеум на възходящия колон, с епиплоитни промени на оментума в частта, произхождаща от възходящото дебело черво и дясната флексура. Възпалителни сраствания бяха открити върху оментума и фундуса на жлъчния мехур, който също беше покрит с фибринови отлагания. Установи се некроза на апендикуларния чукан, която бе разхлабила скобите върху него. Налице бе директна комуникация, свързваща лумена на цекума и перитонеалната кухина през некротизирания апендикуларен чукан. Намери се фибрино-пуролентен секрет, примесен с фекулентен в цялата коремна кухина, но предимно в дясния латерален кант, субхепатално, в малкия таз и дори в субфреничното пространство зад черния дроб, където са открити фибринови отлагания, както при дифузен перитонит. Беше взето решение за нерезекционна хирургия въз основа на факта, че перитонитът и бременността са два фактора, които се допълват комплементарно и влошават състоянието на пациентката и поради факта, че тифлитните промени на цекума не бяха крайно тежки. Двата пластмасови клипси на апендикуларния чукан бяха отстранени. С единични шевове бе затворен дефектът на цекума, причинен от некрозата на апендикуларния чукан. Шевовете на цекума бяха покрити с кесиечен шев на втори етаж. Извърши се лаваж на цялата коремна кухина с антисептици. Всички фибринови отлагания, открити върху париеталния и висцералния перитонеум и покриващи коремните органи, бяха отстранени внимателно. Коремната кухина беше дренирана с шест тръбни дренажа – два дренираха субфреничното и субхепаталното пространство отдясно. Два контактни дренажа бяха поставени в десния латерален кант – горният: по дължината на десния латерален кант, а долният бе разположен в малкия таз, зад матката. Два контактни дренажа бяха поставени от лявата страна на предната коремна стена - горният - в малкия таз, зад матката, а долният – пред матката, зад пикочния мехур. ДИСКУСИЯ Некрозата на апендикуларния чукан е тежко усложнение, водещо до развитие на перитонит, което застрашава живота на пациента. Основните дискусии относно тази патология се фокусират върху причината за нейното развитие. В различни клинични проучвания и рандомизирани изследвания са проучени различни хирургични техники, за да се определи причината за некрозата на апендикуларния чукан. Някои обвиняват исхемията, докато други приписват проблема на недостатъчността на метода за затваряне на апендикуларния чукан, стадия на възпалението и други фактори. Симптомите на некрозата на чукана се проявяват с повторно появяване на болка в долния десен квадрант, чувствителност, позитивиране на обтураторния и псоасния симптом. Тази неясна клинична картина, в комбинация с напредналата бременност и анатомичната промяна в позицията на органите в корема допълнително маскира клиничната проява на това рядко състояние. Освен това, не могат да се прилагат бързи методи за образна диагностика, с изключение на ултразвуковото изследване, което може да се окаже недостатъчно поради уголемената бременна матка. ЯМР не е предпочитан метод на избор поради дългото време за създаване на изображение. (1319) ЗАКЛЮЧЕНИЕ Некрозата на апендикуларния чукан е значително усложнение, което застрашава живота на пациента и води до перитонит. Симптомите се проявяват с повторно появяване на болка в долния десен коремен квадрант, чувствителност, позитивни обтураторни и псоасни симптоми. В последния триместър на бременността клиничната проява на състоянието е маскирана поради анатомичните промени в положението на коремните органи. СП. ХИРУРГИЯ • J. KEYWORDS appendicular stump necrosis, appendectomy, last-trimester pregnancy INTRODUCTION By definition, appendicular stump necrosis or stump appendicitis is a pathological condition in which the leftover appendix fragment after the appendectomy undergoes a process of inflammation or tissue ischemia, resulting in necrosis of that fragment, which can even progress to the caecum. During pregnancy, with the growing fetus, the female organism undergoes rapid, dramatic changes. The change is not only physiological but also anatomical. An enlarging uterus pushes the adjacent organs away. The whole colon, and more precisely the cecum and the appendix, are moved cranially. The standard anatomical position of the organs is altered, which, to some extent, blurs the correct symptoms. As a normal progression, the cecum and appendix are pushed to the level in the right hypochondrium during the last trimester of pregnancy. The most palpable symptoms are not well presented and are masked by the enlarged uterus and the moving fetus inside it. The regular abdominal examination and palpatory signs cannot be implemented. The typical appendicular pain following appendicitis is, in most cases, presented in a new atypical spot. [ 1,2,3,4 ] Appendicular stump necrosis is a common complication that develops in the early postoperative period. It is a rare condition, with an incidence rate of approximately 1 in 2,000 to 2,500 cases. It presents with a recurrence of typical appendicular pain and a high temperature. The condition progresses rapidly into peritonitis. The early manifestation of the appendicular stump necrosis is presented with a sudden onset of pain in the lower right quadrant of the abdomen, right flank of the abdomen, and right hypochondrium, high body temperature, and fever. In most cases, the patients report a recurrence of the main symptoms of appendicitis, as before the surgical intervention. This symptomatic recurrence, in most cases, regardless of its good presentation, is diminished, diluted, and masked by the pain caused by the operative intervention itself. The condition rapidly progresses into peritonitis and becomes a life-threatening condition. [5,6,7,8,9,10,11,12] We present the case of a 30-year-old woman, pregnant at the 31st gestational week, referred to the surgery clinic. She presented with symptoms of pain in the upper abdomen of an undefined nature, which started yesterday. Since morning, the pain has been localized to the lower abdomen and mainly on the right side, and has become constant and accompanied by nausea, without vomiting. Subfebrile up to 37.7 C. Pathology related to pregnancy was excluded, and the patient was admitted to the hospital with clinical and laboratory data for acute appendicitis. The patient presents with an anamnesis of food allergies to nuts and fruits. СП. ХИРУРГИЯ • J. Slightly depressed general condition. Skin and visible mucous membranes - pale. Tongue - moist, coated with whitish plaque. Normostenic habitus. Clear vesicular breathing, weakened somewhat in the right base, no rales. Rhythmic heart activity. Blood pressure - 120/80 mmHg. Abdomen - above the level of the chest (31 gestational week), uterus at the umbilical level, soft. Painful on the right lateral canal with max. At the level of the right hip, there is also evidence of local peritoneal irritation. The palpatory signs of Mendel, Dunphy, Rovsing, and Blumberg were positive. Sluggish peristalsis was auscultated in the whole abdomen—limbs - no edema or trophic changes. In the clinical laboratory, the leucocytic count was higher at 13.33 × 109. The patient was operated on, and a laparoscopic appendectomy was performed. Pneumoperitoneum was established using the Hasson open subxiphoid technique. A 10-mm trocar was inserted under visual control, allowing the introduction of the optics. The examination revealed an enlarged uterus with adnexa, corresponding to the gestational age. A small amount of cloudy serous exudate in the ileocecal region. Aspiration of the effusion. 1. Gangrenous, inflamed appendix. 2. Moderately dilated mesenteric vessels periapical. No other visible pathology was found in the intraperitoneal organs accessible for examination. Another 10 mm trocar was placed in the right hypochondrium, and a 5 mm trocar was placed infraumbilically, and an appendectomy was performed. The mesoappendix was cut using LigaSure. The appendix was dissected to expose its base. The stump was transected with TA 30/3.5 and Ligasure. A safety device was placed on the appendix, and it was removed through a 10 mm trocar in the right hypochondrium. Abdominal ultrasound: three days later showed no pathology. On the fifth day, a sharp pain in the lower right abdominal quadrant recurred. The pain was constant and increasing over time. The nausea recurred again. From the laboratory – new higher in the leucocytes – 10.5x109 and higher than the normal CRP -177.27. The patient was transferred to a specialized clinic of Obstetrics and Gynecology. Due to worsening symptoms, a consultation with an abdominal surgeon was requested. The clinical exam showed a worsening condition with positive symptoms of appendicitis: Mendel, Dunphy, Rovsing, and Blumberg. Even the obturator and psoas symptoms were positive. Only an ultrasound was permitted to be used for diagnostics due to the pregnancy. With a working diagnosis of peritonitis post-appendectomy, a decision was made to interrupt the pregnancy and perform a relaparotomy. After completion of the obstetric stage, a Csection of the uterus was performed, and one live fetus was delivered. The procedure continued with the surgical stage of the intervention. Covering the uterus with antiseptic compresses, exploration of the abdominal cavity was conducted along the right lateral edge. A status post laparoscopic appendectomy was found, with typhlitis and perityphlitis changes in the cecum. The same changes were found on the visceral peritoneum of the ascending colon, with epiploic changes on the omentum in the part originating from the ascending colon and the right flexure. Inflammatory adhesions were found on the omentum and fundus of the gallbladder, which was also covered with fibrin deposits. An appendicular stump necrosis was found, which has loosened the clips on the appendicular stump itself. There was a direct pathological opening connecting the cecum lumen and the peritoneal cavity through the necrotized appendicular stump. Due to this pathological condition, a feculent secretion mixed with a fibro-purulent secretion was found throughout the entire abdominal cavity, but predominantly in the right abdominal flank and even in the subphrenic space behind the liver, where fibrin deposits were found as in diffuse peritonitis. SURGERY 3'2025 Vol.89 Issue3 Online ISSN: 3033-151X, Print ISSN: 0450-2167 https://bgss.eu/ DOI: 10.5281/zenodo.17400315 8 A decision for a non-resectional surgery was made based on the fact that peritonitis and pregnancy are two factors that are self-potentiating and only aggravate the condition of the patient. The two plastic loose clips of the appendicular stump were removed. With a single sutures, the defect of the cecum caused by the appendicular stump necrosis was closed. The sutures of the cecum were covered with a purse-string sero-serous suture. A lavage with antiseptics was performed in the whole abdominal cavity. All the fibrine deposits that were found on the parietal and visceral peritoneum and covering abdominal organs were removed gently. The abdominal cavity was drained via six drainage tubes – two draining the subphrenic space and one draining the subhepatic space on the right. Two contact drains were placed on the right side of the abdominal wall. Distally located - the upper one: along the right lateral edge. The lower one is located in the lower pelvis, behind the uterus. Two contact drains were placed on the left side of the anterior abdominal wall: the upper one in the lower pelvis, behind the uterus, and the lower one in front of the uterus, behind the bladder. DISCUSSION Appendicular stump necrosis is a significant complication that threatens the patient's life and can lead to peritonitis—the primary discussion regarding its development centers on the cause. In various clinical trials and randomized studies, different surgical techniques have been investigated to determine the cause of appendicular stump necrosis. Some blame the ischemia, while others attribute the issue to the insufficiency of the appendicular stump closure method, the stage of inflammation, and other factors. Symptoms of the stump necrosis are present with a reappearance of the lower right quadrant pain, tenderness, obturator and psoas sign. This elusive clinical picture, in combination with a late-term pregnancy, occurs in a state of anatomical organ positional change, which further masks the clinical manifestation of that rare condition. As a primary goal is to protect the fetus, no X-rayutilizing methods like a CT scan can be used. Furthermore, no rapid imaging methods can be used except ultrasonography, which can be insufficient because of the enlarged pregnant uterus. MRI is not the method of choice due to its lengthy image creation time. [13-19] CONCLUSION Appendicular stump necrosis is a significant complication that threatens the patient's life, resulting in peritonitis. Symptoms of the stump necrosis are present with a reappearance of the lower right quadrant pain, tenderness, obturator and psoas sign. In late-trimester pregnancy, the clinical manifestations of the condition are masked by anatomical changes in the position of the abdominal organs. СП. ХИРУРГИЯ • J.
