ORGANIZATION OF PATRONAGE SERVICES FOR PATIENTS WITH CARDIOVASCULAR DISEASES AND IMPROVEMENT OF PRIMARY HEALTH CARE DELIVERY
Abstract
Cardiovascular diseases (CVDs) are globally recognized as one of the leading threats to human health, accounting for more than 17 million deaths annually worldwide (World Health Organization, 2024). In Uzbekistan, these diseases significantly impact public health, with the proportion of chronically ill patients steadily increasing. This situation necessitates long-term follow-up, individualized approaches, and strengthening of home-based patronage services. Improving primary health care (PHC) delivery ensures the development of a systematic and integrated model of continuous medical supervision.

Improving primary health care (PHC) delivery ensures the development of a systematic and integrated model of continuous medical supervision. Introduction. To assess the role and significance of patronage services in improving the health of patients with CVDs, to analyze their interaction with primary health care, and to develop an effective organizational model based on advanced international experience. Because, despite advances in diagnostic and treatment technologies, the prevention and long-term management of CVDs remain insufficiently effective. Materials and Methods. The study analyzed 420 patients with CVDs observed between 2021–2024 at the Republican Cardiology Center and urban and district family polyclinics. Comparative analysis was conducted using scientific literature, statistical data, and foreign studies (Johnson, 2021; Lee et al., 2023; Patel, 2024). The main methods included analytical assessment, monitoring, sociological surveys, and evaluation of digital patronage platforms. Results and Discussion. Significance of Patronage Services. Home-based patient monitoring, control of medication adherence, and provision of recommendations on nutrition and physical activity significantly contribute to preventing complications. Practical observations demonstrated that patients under regular patronage supervision showed a 25–30% reduction in hypertension and ischemic heart disease recurrences (Smith et al., 2022). Challenges in Primary Health Care. The findings indicate that preventive approaches among local physicians in managing CVDs remain insufficiently automated. Electronic health records and digital data systems are underdeveloped, resulting in delayed data processing and inefficient coordination of care. Opportunities of Artificial Intelligence and Digital Technologies. In international practice, AI-based care management systems allow physicians to monitor patients’ conditions in real time and receive automatic alerts regarding potential deterioration (Wang, 2022). Integrating such technologies into local patronage systems can minimize human error and improve the timeliness of medical interventions. Human Resources and Pedagogical Orientation. For effective patronage service delivery, it is essential that doctors and nurses develop digital literacy, data security awareness, psychological competence, and modern communication skills (Azimova et al., 2023). Specialized educational modules in these areas should be introduced in medical colleges and universities. Social and Managerial Importance. Enhancing collaboration between community doctors, patronage nurses, and local residents is critical for the development of primary health care. Strengthening this interaction enables early detection of diseases and promotes a culture of healthy living among the population. Conclusion. To improve the health outcomes of patients with cardiovascular diseases, it is essential to reorganize patronage services using modern technologies. Digitalization of primary health care, implementation of artificial intelligence, enhancement of staff competencies, and promotion of healthy lifestyle awareness should become national priorities in public health policy. This integrated approach can significantly reduce CVD-related mortality and disability while enhancing the overall efficiency of the healthcare system.
