MORPHOLOGICAL CHANGES IN COATED ND-YAG LASED ENAMEL DR. LUIS CORPAS PASTOR Y DR. AUGUSTO ELIAS BONETA ABSTRACT Objective: The purpose of this study is to evaluate the enamel surface alteration microscopic patterns after laser etching by using different laser energy output and laser initiators. Design: This is an observational study of dental enamel anatomy surface alterations by using different quantitative and qualitative variables ─respectively, two laser energy output and two dye primers (waterproof black ink and black pencil)─ with scanning electronic microscopy (s.m.e.). Materials and Methods: Four maxillary human permanent premolar teeth, extracted stored in saline water, free of hypoplastic areas, cracks, or gross enamel irregularities were chosen. Interventions: The roots of the teeth were sectioned from the coronal portion and cleansed ultrasonically in deionised distilled water (DDW) for 5 minutes and stored in DDW, at 37oC. Each coronal portion was divided in two specimens and either acid or laser treated. Main Outcomes Measures: Enamel etching was performed by using an Nd:YAG laser (American Dental Laser) with pulsed energy type and variable power (up to 3 W.) delivered by fibre-optics. Every specimen underwent a different etching procedure, testing two different “laser initiators” (waterproof black ink and black pencil). Specimens were immersed in tridistilled deionised water, rinsed, and sputtered with a layer of gold in an "ETEC gold sputtering BIO RAD, Mod. E 5200", by using 18 mA. for 100 seconds. Morphology of each specimen was evaluated by s.e.m. Results: The highest laser doses tried (20 Hz. / 1.50 Watts), by using both primers, causes variable width pits and large craters. However, double laser exposition at 15 Hz./ .75 Watts, with both primers, causes fused enamel with melting and many pits of variable diameter, giving a drilled aspect. There are some microfractures that seems to be narrower than the ones obtained with the 20Hz./ 1.50 dosage. Higher number of pits and narrower microfractures appears by using black ink as laser primer. Conclusions: The Nd:YAG laser irradiation of enamel using a dark organic substance as “laser initiator” produce ─under conditions used in this study─ the ablation of enamel, with the apparition of cracks, microfissures and micropits in irradiated enamel areas, which could be the a starting point of future caries development. Thus, more research should be suggested about the clinical relevance of those laser induced microfractures before using laser to promote adhesion in orthodontics. Diseño: Estudio observacional de las alteraciones de la superficie de la anatomía del esmalte dental mediante el uso de diferentes variables cuantitativas y cualitativas ─respectivamente, dos salidas de energía láser y dos imprimaciones de tinte (tinta negra resistente al agua y lápiz negro) ─ con microscopía electrónica de barrido (s.m.e.). Materiales y métodos: Se seleccionaron cuatro dientes premolares permanentes humanos superiores, extraídos almacenados en agua salina, libres de áreas hipoplásticas, grietas o irregularidades gruesas del esmalte. Intervenciones: las raíces de los dientes se seccionaron de la porción coronal y se limpiaron ultrasónicamente en agua destilada desionizada (DDW) durante 5 minutos y se almacenaron en DDW, a 37 ° C. Cada porción coronal se dividió en dos muestras y se trató con ácido o con láser. Principales medidas de resultados: El grabado de esmalte se realizó mediante el uso de un láser Nd: YAG (American Dental Laser) con tipo de energía pulsada y potencia variable (hasta 3 W.) suministrado por fibra óptica.
